I think Thomas was abused. I think the abuse traumatized him and even though he was "born deaf" it was kind of selective. He may have been born autistic, and they can be rigid in their routines and Thomases routine could've been "selective deaf"...?  (I'll read on before I make that official.)
Were they close to Chernobyl, within range of radiation damage/sickness?
Why is her joyous moment described as "slicing her open"?

Unbelievable the emotional roller coaster that Rebecca went through and then "snapped back into place" when Tomas suddenly reappeared.
This was too long not to have a definitive conclusion. Some way of letting us know what actually happened to Tomas?  This woman is weird the way it leaves us hanging but still technically was a happy ending.
